She has our Professional Listing. PHR forms are printed and available in the Case Manager office on 2W – (Just outside the 2W main doors).

Please take note of the Projects we have coming forward:

  1. Transitions Coach program still ongoing. We have been keeping up with what is being done on a national level. Keep practicing on those in your congregation when discharged.
  2. Rural Aging Initiative Grant. This Grant focuses on our aging populations and keeping those we serve in their own homes as long as possible. Advance Care Planning and Mental Health are the 2 most prevalent tasks that health systems and cities are working on collectively. Hopefully through Grant funding we can have training in both of these areas offered to YOU! Keep this project in your prayers.

Advance Care Planning 1-day training set for Thursday Feb. 22 8am to 4:30pm at St. Mary’s Hospital in Superior. Register By Feb 10th contacting barb.krause@essentia.org

Our organization has been given 5 spots for the training – Please contact Barb Krause at Essentia if you are interested in this BY Feb 10th.  It will be of no cost to you. There are some volunteer requirements, of which you most likely will be doing in your congregation OR if you offer community events through your congregation.  I did this training about 18mos ago and it has been very helpful especially in combination with the “Through the Wilderness” – Dementia Training. You will be given ALL the resources you need.  For the Initial Requirements, below, there are online modules so you can prepare for the day.

Initial Certification Requirements

  • Completion of the FS ACP Facilitator Certification Online Curriculum (Modules 1-4)
  • Completion of the FS ACP Facilitator Certification classroom training (8-hour classroom instruction)

Ongoing Certification Requirements

Currently, RC (Respecting Choices) has no ongoing recertification requirements.

The Duluth Advance Care Planning Council is looking for persons who will agree to be trained and be available to assist with advance care planning in the community.  This may include facilitation with both groups as well as individuals.  We are looking for persons trained as facilitators to be available for a minimum of two facilitations per month for one year.

If you, as the parish nurse are not able to attend – but there may be a deacon or pastoral or lay staff interested – please share this information and have that person register.

Leadership Training for Health Promotion Workshops: JUNIPER WEBSITE

The Arrowhead Area Agency on Aging is seeking professionals or volunteers to become leaders to facilitate groups of people with chronic health conditions or chronic pain through Living Well Workshops. Living Well with Chronic Conditions and Living Well with Chronic Pain workshops help people manage their symptoms, build confidence, and maintain an active lifestyle.

People interested in becoming a leader will complete a 4-day classroom training for Living Well with Chronic condition, with the option of an additional 2-day cross-training for Chronic Pain. The next 4-day Chronic Conditions leader training opportunity is February 22, 23, 26, and 27 from 8:30am to 5pm in Duluth. The 2-day Cross Training will be held February 28 and March 1, from 8:30am to 5pm in Grand Rapids. The Deadline for Leadership Training Registration is February 16th and is limited to 16 potential leaders. The cost is $200 for Chronic Condition training and $100 for the Chronic Pain Cross Training. Limited scholarships are available upon request. Contact Hours for our RN license renewal are now 60 minutes in length rather than 50.  Also can count in 15 minutes increments. It came from Melissa Wolfe, MN Board of Nursing – this was changed in October 2017, the MN Board of Nursing adopted new rules re: CEU’s.
