PROGRAM:   CSS Post-Bac Nursing Students have developed a Pre/Post Test for us for our Dementia Project and also Objectives for the series.  They will be our Guest Presenters:  Lauren Johnson, Danielle LeMasurier, Sarah McIsaac, Justyna Obrzut, Nicole Salane, Allison Schute.

From North:  Take 53 into Duluth. Left on Arrowhead Rd (just past Menards) – and take it to Woodland Ave . Make a Left on Woodland, and Right on Snively (which is the light right after you turn). Take Snively for about 1mile. The Church will be in front of you on the Left Corner.

Care Transition Program – –

Please Make sure your congregates know that YOU are their Parish Nurse. When you visit them in the hospital or nursing home settings feel free to leave a calling card OR write your name on the patient “white board” in their room.  I hope that you have had a chance to “listen-in” on the conference calls, as there has been some good tips and info shared.  Remember, our St Luke’s Contact is Lois Opseth RN- Discharge Advocate in the Discharge Planning office 249-5699 OR lois.opseth@slhduluth.com, Outside beeper number 218-249-8020.  She has our Professional Listing. PHR forms are printed and available in the Case Manager office on 2W – (Just outside the 2W main doors)

  1. With the Rural Aging Initiative Grant (Northland Foundation) – we are able to provide a Scholarship for Parish Nurse Preparation – Please pass this on!!

  1. Transitions Coach program still ongoing- – we are keeping up with what is being done on a national level. Keep practicing on those in your congregation when discharged.
  2. Our “Live Well at Home” grant from Dept. of Human Services for a Dementia and Communities of Faith (training and program tools).You will be asked to report on the screening tool results each quarter. Please know that your participation is very valuable in this grant process. You should have received your church’s copy of “Through the Wilderness” book and curriculum workbook by now. If not – please let me know.   PLAN now for the fall in presenting this to your adult groups in your congregation.  Add an Advance Directive opportunity at the end, or as a follow up.

May Meeting Highlights

Jenny Peterson from Generations led us in a small focus group session looking for community input from cross section on what is “working” – what is not. AND what are the areas of great concern.  Very good discussion as well as input into this community process. I will have copies of the slides that  she presented at the next meeting.

Northland Foundation – Rural Aging Initiative Grant Monies Re- Directed.

You should have all received your 2014 Transition Coach Booklet. The assessment tools and checklists looked very valuable when I saw this at the Westberg Symposium in April. I hope and trust it will serve you well in your practice.

Please NOTE that we also have funds for RN’s who wish to take the Parish Nurse Preparation course (in classroom or online settings); OR if YOU would like to attend a Conference/Training that you do not have funds for – – PLEASE let me know.   I believe there will be some Advance Care Planning Trainings coming up in the late summer/early fall. If you have an interest in this, the funds could certainly go towards this as well.
