This Month our meeting will be on Thursday November 10th from Noon to 2:30PM at BAYSHORE Health Care/Rehab Facility, 1601 St Louis Ave, Duluth on Park Point.

Guest:  Georgia Lane, LGSW, Program Developer from Arrowhead Area Agency on Aging will introduce us to the Chronic Disease Management Classes that we can be trained for OR learn how to access them.

Program will be provided by staff at Bayshore.

DIRECTIONS:

From South: Take Hwy 35N to the Lake Ave exit – Travel through Canal Park onto Park Point. Bayshore is on “bay-side” watch for signs at 15th Street.

From North: Take 53 into Duluth. Go past the Mall follow Central Ave to Lake Ave all the way to Park Point – see above for directions.

National Parish Nurse News:

The Westberg Symposium will not be held in ONE location next year – instead they will be offering the Symposium across the county in different regional locations.  MN had tried to be a host, but it just did not work out.  Our regional location will be held in Lansing, MI – April 21 & 22. Mark your new calendars now!

 

Care Transition Program – –

I hope you have had a chance to listen-in on the conference calls; there has been some good tips and info.  We have a new contact (newly hired) and I will find out who she is. She is the Discharge Advocate in the Discharge Planning office 249-5699.  PHR forms are printed and available in the Case Manager office on 2W – (Just outside the 2W main doors)  Even if you have people admitted to Essentia or other hospital systems – keep up your skills by being a Transitions Coach to them.

Please take note of the Projects we have coming forward:

  1. With the Rural Aging Initiative Grant (Northland Foundation) – we are able to provide a Scholarship for Parish Nurse Preparation – Please pass this on!!

  1. Transitions Coach program still ongoing- – we are keeping up with what is being done on a national level. Keep practicing on those in your congregation when discharged.
  2. Our “Live Well at Home” grant from Dept. of Human Services for a Dementia and Communities of Faith (training and program tools).You have been asked to report on the screening tool results each quarter. Please know that your participation is very valuable in this grant process. You should have received your church’s copy of “Through the Wilderness” book and curriculum workbook by now. PLAN now for the fall in presenting this to your adult groups in your congregation. Add an Advance Directive opportunity at the end, or as a follow up. If you need a copy of the Pre/Post Tests – please let me know.  See the Tools/resources we received at the last PN meeting. Attached and included in this issue.

Parish Nurse Preparation Course

This is news from Northern Waters PN Ministry – – The Wisconsin Nurses Association is sponsoring a parish nurse preparation course on March 17, 18, 31 and April 1 2017. Let Pam know if you are interested in this course or would like to have a prep course offered in NW WI next year.  Jan Stevens will be contacting the organizers.

 

November Spiritual Companion Series this year will be: : Keep Your Headlights On; Respectful Care for People Living with Depression, Anxiety & Addiction at End of Life. Exploring the importance of helping you identify spiritual distress and mental illness at the end of life. (All locations will be 8:30 am to 12:30pm) The Email address beside each location is where you can RSVP.  This is FREE!!

ANNIVERSARY RECAP – –

Thank you so much to ALL who participated in the 25th/20th Anniversary Party for the Parish Nurse organizations on either side of the “bridge”!  There were over 100 of us current/retired and guests.  I hope that you found the evening renewing and encouraging.
