This event is being held during Mental Illness Awareness Week. Please make plans to attend and bring your pastor(s) and lay leaders in preparation of being able to serve those with this type of chronic illness. Make your reservations TODAY! As it is FILLING UP FAST!This is being sponsored by a grant from the Miller Dwan Foundation and we are working together with NAMI (National Alliance of Mental Illness – Duluth Chapter)
Tuesday Evening 6pm Registration/6:30 to 8pm will be the De-Escalation Training.
Wednesday is specifically geared towards communities of faith; there will be a Pastor on the Panel Presentation for Suicide Pre-and Post-Vention.
8:15am registration/workshop begins at 9am ends at noon. (3 CEU’s)
All events will be held at the Inn on Lake Superior; 350 Canal Park Dr.

She is the Discharge Advocate in the Discharge Planning office 249-5699. PHR forms are printed and available in the Case Manager office on 2W – (Just outside the 2W main doors)

- Transitions Coach program still ongoing- – we are keeping up with what is being done on a national level. Keep practicing on those in your congregation when discharged.
- Our “Live Well at Home” grant from Dept. of Human Services for a Dementia and Communities of Faith (training and program tools).You have been asked to report on the screening tool results each quarter. Please know that your participation is very valuable in this grant process. You should have received your church’s copy of “Through the Wilderness” book and curriculum workbook by now. PLAN now for the fall in presenting this to your adult groups in your congregation. Add an Advance Directive opportunity at the end, or as a follow up. She explained her role as Care Consultant and also the services that are provided. In following up on referrals, she does a home visit and helps people navigate through the caregiver role. She can also give you the care consultant in your area – if you are not in her service area.

In her role of Volunteer Coordinator she is not only connecting volunteers to the services needed by participants – She is also looking for volunteers. BELOW is some info for bulletin.
Age Well Arrowhead is a non-profit organization in Duluth that connects older adults (60+) and caregivers to services that promote healthy aging and independence. We are looking for volunteers to help with household services, work in the yard, mow or shovel snow, shop for & deliver groceries or provide transportation to a medical or essential well-being appointment. Volunteer as often as you’d like. We are flexible and able to accommodate your schedule.
When you serve with Age Well Arrowhead, you’ll learn that even the smallest task can make a big difference in someone else’s life! At the end of the day, there are few things as rewarding as the genuine gratitude shown by the older adults we care for.

He also gave us several Document/TOOLS to help those that we serve get their finances in ORDER – especially with early onset.
