Care Transition Program – –

I hope you have had a chance to listen-in on the conference calls; there has been some good tips and info.  Our St Luke’s Contact is Amanda Klinghoffer RN- Discharge Advocate in the Discharge Planning office 249-5699. She has our Professional Listing. PHR forms are printed and available in the Case Manager office on 2W – (Just outside the 2W main doors).

Please take note of the Projects we have coming forward:

  1. Transitions Coach program still ongoing- – we have been keeping up with what is being done on a national level. Keep practicing on those in your congregation when discharged.
  2. We are in the middle of Northland Foundation’s Rural Aging Initiative Grant; focused on Advance Care Planning as well as Mental Health “Make it Okay”
  3. We recently were awarded the Operational Grant we requested from Essentia Health for $15,000.

Our NOVEMBER Meeting Update

At our November meeting we had 2 great “links” to information and services!

      Kristy Lundberg from Essentia Health – – Kristy is the Osteoporosis Nurse in the Rheumatology Dept there. I am sure a lot of us had a connection with her at one time or another. Osteoporosis is a Silent disease.  Did you know that MEN as well as WOMEN can have Bone Loss?   Major risk factors of Bone Loss: Lifelong Low Calcium/Vit D,  Current Tobacco use, Alcohol >2drinks daily, and Lack of Exercise.  Our roles as Nurses: **Encourage Prevention (eliminating the Major risk factors- above)

**Get adequate amounts of Calcium and Vit D,  **Stop smoking, **Get 30 minutes of weight bearing activity/day. Resistance exercise 2-3 days a week.  **Fall Prevention (keep walkways clear!), ** And have a DXA Scan.

A good follow up to Falls Prevention.
