This Month our meeting will be on Thursday January 10th from Noon to 2:30PM at  FIRST COVENANT CHURCH,  2101 W 2nd Street, Duluth (218-722-5451)  Bring a Sack Lunch – dessert/beverages will be provided.

Program :  Elizabeth Henderson, Program Coordinator for the Duluth Mental Health Court will be our presenter about this program: The Program’s mission is to improve mental health, promote self-sufficiency, reduce recidivism, and offer cost-effective alternatives to incarceration and hospitalization for participants.

Following this presentation:

Megan Helena, Program Director from Generations Health Care Initiatives will lead us in a “Round-Table” discussion on looking at Community Health Needs and Gaps in this area of MENTAL HEALTH.  In Preparation for this discussion – Please have a discussion with your Pastoral Staff and congregates to get a “Congregational Perspective” on gaps in services for this area of Health.   Thank you!

From South: Take Hwy 35N to the 21st Ave West Exit (Left lane). Then staying in the Right lane, circle around under the freeway – the Church will be on your LEFT.

From North: Take 53 into Duluth. Go past the Mall, and at Trinity Rd make a Right (follow signs to 35W) coming down Piedmont Ave – exit at the 21st Ave West ramp. The Church will be directly in front of you.

In Duluth: the church is on the corner of 21st Ave West and 2nd street, across from Mid-Town Manor in Lincoln Park.

Care Transition Program – –

I hope you have had a chance to listen-in on the conference calls; there has been some good tips and info.  Our St Luke’s Contact is Amanda Klinghoffer RN- Discharge Advocate in the Discharge Planning office 249-5699. She has our Professional Listing. PHR forms are printed and available in the Case Manager office on 2W – (Just outside the 2W main doors).

Please take note of the Projects we have been a part of:

  1. Transitions Coach program still ongoing- – we have been keeping up with what is being done on a national level. Keep practicing on those in your congregation when discharged.
  2. Advance Care Planning – Advance Directives may be on someone’s resolution list!!
  3. Mental Health “Make it Okay” and Through the Wilderness:  plan a presentation for your congregation this year on these valuable learning tools.
