I hope you have had a chance to listen-in on the conference calls; there has been some good tips and info.  Our St Luke’s contact is Amanda Klingelhoffer RN- Discharge Advocate in the Discharge Planning Office 249-5699. She also has our professional listing. PHR forms are printed and available in the case manager office on 2W– (just outside the 2W main doors).

Please take note of the projects we have in place:

  1. Rural aging initiative grant. This grant focuses on our aging populations and keeping those we serve in their own homes as long as possible. Advance care planning and mental health are the 2 most prevalent tasks that health systems in our cities are working on collectively. Through this grant funding we have offered  you training in both of these areas!  Our May meeting was a training in Make it Okay through the NAMI Educators.
  2. If you or any of your  RN friends that might be interested in this health ministry – please let them know we have Scholarships Available to assist with the cost of the Preparation Course (trainings)

June Meeting Recap – Dementia Friends:

Thank you to those that participated in the training we had last month. Cindy Conkins and Rebecca Sash did a great job of leading us through the training on what we can offer our congregations . Penny shared that she has incorporated this in her “Through the Wilderness” Presentations to her congregation.

People with Dementia need to be understood and supported in our community.   You, me and all our friends and neighbors can help by becoming a Dementia Friend!

What Makes You a Dementia Friend? You become a Dementia Friend by attending a one-hour informational session to learn what dementia is, what it’s like to live with the disease and how to turn your understanding into action that supports people living with dementia.

What Happens at the Information Session? The one-hour session is a face-to-face discussion led by a Dementia Friends Champion; it’s not a formal training or education session. The session is informal and includes activities and interaction. You’ll learn 5 key messages about dementia and tips for communicating with people who have dementia. Everyone who attends is asked to consider a practical action that can help someone in their community. The action can be big or small — every action counts!

The cost of the week-long course remains at $450.00 which covers the cost of meals and most materials to start a health and healing ministry. Foundations for Faith Community Nursing Course offered by Sanford Health

September 14-November 19, 2018

Sanford health offers the foundations for faith community nursing certified course taught by professional nursing faculty who are experienced faith community nurses representing an array of faith traditions. The course is a pre-requisite for RNs who want to enter faith community nursing practice.

This course explores the specialty practice of faith community nursing that combines professional nursing and health ministry, emphasizing health and healing within the faith community and the larger community. It offers registered nurses with a strong spiritual faith the knowledge to implement and facilitate a faith community nursing ministry, as well as practical opportunity to apply learnings from the course to the dynamic nature of their faith community and its particular needs. This course derives from the curriculum endorsed by the Westberg Institute for Faith Community Nursing.

The course starts with a weekend retreat near Watertown SD then continues on-line on Monday evenings. Course cost is $390 for RNs not employed by Sanford Health. This fee covers the cost of the retreat, books and other course materials.
